Description
The participant of this seminar will have an overview of various techniques and methodology used to perform failure analysis. This course will focus on technical and engineering failures but the methodology discussed is not limited to technical subjects, it has a wide range of applications, from business, medical, human performance, to non-technical issues. The final goal of any failure analysis is to identify the root cause of the failure under investigation and present effective mitigating actions to prevent the failure from reoccurring again.
The basic technique will be presented and case studies will be discussed. Participants will involve in hands on workshop session to demonstrate their skill in problem solving.
By the end of the seminar, the participant will be able to use this technique to lead a multi-discipline, cross-functional team to execute a successfully failure analysis.
The course material are targeted to individuals with no or limited background on the subject. It is also presented as a refresher course for people who have prior experience in failure analysis.
 
Objectives
The Objective of this course is to provide the basic knowledge and tools required to identify the root cause of a failure and to present a solution.
A practical, systematic and non theoretical approach is used to guide the participants through out this course. Starting from identifying the issues and formulate the problem statement, define the term of reference, listing all possible causes of the failure, identify the root course and all contributing factors. The final output will include recommended mitigating actions and identify trending metrics to measure success.

Individuals who are involved or interested in problem solving and to analyze why systems fail.  
Managers, engineers and technologists, maintenance and operation personnel and all technical or non technical personnel who need to learn, upgrade or refresh their knowledge in problem solving and to conduct failure analysis.

  • Understand the various methodology used in problem solving and failure analysis. To conduct and co-ordinate a multi-discipline team to investigate the problem and present mitigating actions.

Gee Sham

Gee Sham P.Eng. Senior Engineer with former Bechtel International, former Ontario Hydro and various consulting companies, bringing with him in the area of piping design, layout, analysis and operation for Process and Power plants.

He has provided expertise as a consultant and acted as lead design engineer in mining, food processing, petro-chemical and power plants in Canadian and International projects. In addition to the design and construction phase of the project, he was also the project lead in system commissioning and plant start-up activities.

Gee Sham also has many years of experience in operation and maintenance of process and power plants. The main areas include system performance improvement and leading multi-discipline teams to perform troubleshooting, root cause analysis and failure analysis. In addition, he has work as a project engineer to execute design and modification in Mechanical and Instrumental and Control projects.

As an active instructor in this field, he has been involved in industrial training for over 25 years. He has taught courses in technical colleges, in-house training for consulting companies and offered lectures in various technical training institutions. Students from across Canada and overseas have participated in these courses and have provided very positive feedback.

Initial stage

  • Problem Identification
  • The terms of reference
  • Team approach and identify resources
  • Setting the ground rules

The use of various techniques

  • Investigation Matrix
  • Information gathering and categorization. (probabilistic verses deterministic analysis)
  • Potential failure modes/ Failure Mode analysis
  • Event and Causal factor/Fishbone chart
  • Pareto Analysis

Analytical phase

  • Key probable failure modes and review similar past failures
  • Define failure Scenario and recreation of the failure
  • Identify the root cause and other contributing factors
  • Reality check

Providing the Solution

  • Provide the path forward/recommend mitigating actions
  • Risk Analysis/metrics to be trended
  • Cost and benefit analysis/sensitivity analysis
  • Interim and Final report
 
Day II
Case study 1
  • Using a simple case study to demonstrate the use of various techniques

Case study 2

  • Using a more complicated case study to illustrate the approach to root cause identification and multiple contributing factors in a failure analysis.

Case study 3

  • Demonstrate a multidiscipline team approach to problem solving
Discussion of various engineering failures and failure mechanisms.

Review of the various technique used.

Workshop session and final discussion.
